What happened
Anthropic published research into unintended actions by models operating as agents. The question is not only whether a system can follow an instruction, but also which intermediate steps it chooses, what assumptions it makes, and whether its behavior can unexpectedly affect people, data, or external services.
How it works
This matters more as models gain tools and permission to act for a user. An incident may come not from one obviously bad answer, but from a chain of individually plausible decisions that leads to an unwanted result.
Context
The practical lesson is to build autonomy in stages. Systems need scoped permissions, action logs, confirmation for high-risk steps, edge-case testing, and a way to stop execution.
Why it matters
Agentic systems need tests for side effects, permissions, and ambiguous context, not only task completion.
